Vorsicht mit der gleichzeitigen Einnahme von Artemisinine und manchen Antibiotika!

Ein Wirkstoff von AA (Artemisinin) induziert offensichtlich die Aktivität des Cytochroms CYP3A4 und kann zu einer verstärkten Elimination des AB und somit zu einer Senkung des Blutspiegels führen. Damit besteht die Gefahr, dass die minimale Hemmkonzentration nicht erreicht wird und das AB unwirksam wird!

In conclusion, our data showed QHS drugs are potential inducers of P450 enzymes, and the most inducible are CYP2B6 and CYP3A4, which are believed to be the main enzymes involved in the autoinduction of QHS drugs. Several main UGT enzymes and transporters were not induced by QHS or DHA. QHS also showed weak inhibition of CYP2B6, and it did not show TDI of CYP2B6 or CYP3A4. The induction of CYP2B6 and CYP3A was characterized for QHS, and this provided evidence for withdrawing monotherapy and recommending ACTs. Because the DDI prediction showed that QHS could result in a net induction rather than inhibition, the combined drugs (as well as QHS) that are cleared by CYP2B6 and CYP3A4 may require a dose increase to maintain therapy.

Subtrate von CYP3A4 sind u.a. Clarithromycine (aber nicht Azithromycine!) , Erythromycine und Metronidazol (Tinidazol?).

Die gleichzeitige Einnahme von Artemisinine mit diesen AB kann somit zu einem unzureichenden Blutspiegel dieser AB führen!

Ob Tinidazol Substrat von CYP4A3 ist, ist mir nicht bekannt.

Tetrazykline werden m.E. nicht über dieses Enzyms metabolisiert.

Ob die Einnahme von Artemisia annua den gleichen Effekt auf CYP3A4 hat, ist fraglich. Zum Einem enthalten die meisten AA Blätter nur geringe Mengen an Artemisinine (laut einer Studie ca. 0,8% des Trockenanteils der Blätter), zum Anderen ist Artemisinine im Wasser (also auch im Tee?) wenig wasserlöslich. Der genaue Anteil an Artemisinine im Tee ist mir gegenwertig allerdings unbekannt.

Dazu zeigte eine Studie mit AA Tee im Gegenteil zu Artemisinine eine Hemmung von CYP3A4 in vitro bei menschlichen Zellen:

Substanzen, die CYP3A4 hemmen, führen im Gegenteil zu einer Akkumulation von Substraten dieses Enzymes wie Clarithromycine und Metronidazol, also zu erhöhten Blutspiegeln!

Ob das regelmäßige Trinken von AA Tee zu einer Induktion oder Hemmung von CYP3A4 führt und somit zu einem verringerten oder erhöhten Blutspiegel von AB wie Clari/Metro führt, wurde m.E. an Menschen nie untersucht, bleibt also fraglich.

Aufgrund der vorherigen in vitro Studie scheint es eher unwahrscheinlich zu sein.

Borrelioseärzte wie Dr.Hopf-Seidel oder Dr.Klemann verschreiben AA (als Pulver) parallel zu Antibiose als Ersatz zu Hydroxycholoroquin (lysosomotrope Substanz = zur Alkalisierung des Zellinnere), m.E. als Dauereinnahme.
